Living Well with Chronic Disease

Living Well with Chronic Disease is an evidence-based self-management workshop originally developed at Stanford University.

In each session, participants set a realistic weekly goal and create an action plan to achieve it. At the following workshop, they report on their progress and receive supportive feedback from the group to help address any challenges.

Participants learn practical techniques to manage common concerns, including:

  • Coping with the physical and emotional effects of chronic disease
  • Incorporating regular exercise, good nutrition, and proper medication use
  • Communicating effectively with family, friends, and healthcare professionals

 

Who is this workshop for?
Anyone age 19 or older who is living with one or more chronic health conditions—or caring for someone who is.

Participants frequently report experiencing less pain, increased energy, reduced fatigue, fewer healthcare visits, and a slower progression of their illness.
